This report should be composed no later than May 15 of the current school year by the advisor or the outgoing chapter president.  Questions about the form should be addressed to Dr. Jeffrey Carter, National Chair for Youth & Student Activities through the ACDA contact form.  Select the "Student Chapter Information" option on the contact form.
